SUBJECT:    Cafeteria Plan/Supplemental Insurance Open Enrollment

Open enrollment for Section 125/Cafeteria Plan is April 1 thru May 31, 2008. Section 125/Cafeteria Plan allows employees to voluntary elect to have qualified premiums deducted from their paychecks before taxes are deducted. Employees interested in being enrolled in the cafeteria plan are not required to purchase any additional supplemental insurance. During this time, all employees will also need to make some very important decisions regarding their supplemental insurance.  Anyone wanting to add, change, or discontinue their supplemental insurance can do so.  All changes made during this time will become effective July 1, 2008. The changes made during that time will run for a full plan year from July 1, 2008 until June 30, 2009.

Employees interested in adding coverage or changing coverage will contact the appropriate company and make those changes with the agent directly.  Employees wishing to cancel coverage can complete the attached “Cancellation Form” and drop it off to the Office of Human Resources on or before May 31, 2008.  Employees choosing to continue their current deductions as they are will need to contact our office to complete an Enrollment/Election form. 

Agents will be available on campus on April 21, 2008 from 8:00 a.m. to 5:00 p.m. in the Cotillion Ballroom in the Student Union as well as other campus locations during the open enrollment period.  A list of supplemental insurance companies that are authorized to provide coverage through payroll deduction is attached.  Each company’s name, agent, phone number, and available policies are listed.  You may contact the agent listed for in-depth details regarding each policy.
