Overview of Process to Receive Student Accommodations

  • Students must self-identify with the Disability Services Coordinator and formally request accommodations by completing the Application for Student Accommodations and providing any supporting medical documentation. Accommodations are not retroactive, and the University is not obligated to provide accommodations until the application process has been completed and accommodations have been officially approved.
  • Students must meet with the Disability Services Coordinator and engage in the interactive process, which involves a discussion of the accommodations.
  • Students must meet with the instructor to discuss the approved academic accommodations.
  • Students must renew their accommodation request by completing the Application for Student Accommodations.

Pregnant and Parenting Students

The Disability Services Policy covers pregnant and parenting. Additionally, students seeking adjustments due to pregnant and parenting status must complete the Application for Student Accommodations.

Application for Student Accommodations 

This form is for students to apply for academic accommodations.

Disability Services Formal Grievance Form

This form is for formal grievances by students for disability discrimination under the Disability Services Policy. This includes disagreements regarding a requested service, accommodation, auxiliary aid, or modification of a System practice or requirement; denial of a request; inaccessibility of a program or activity; or a violation of privacy in the context of a disability.

Disability Verification Form

This form is to be completed by the medical provider.
