Policy Submission Packet

Instructions

The Policy Submission Packet consists of: (1) Policy Executive Summary; (2) Policy Template; (3) Policy Routing and Checklist Form.

All three forms must be completed and submitted via email to the System Policy Committee members for review and approval. Please Note: All Revised Policies must show STRIKETHROUGHS identifying changes to the original policy when you complete the policy template.
